Home and Community Based Services Site Program

This program provides 24 hour support to adults with disabilities who are eligible for the Home and Community Based Service (HCBS) program. Currently there are six homes in various locations around Fort Dodge, each with four roommates. Because of HCBS rules, emphasis is placed on consumer rights, informed choice, and quality of life. Staff also provides independent living skills training as identified by the consumer’s interdisciplinary team. Direct support staff employed in these homes work seven-day shifts.
 

Residential Care Facilities

There are two five-bed group homes in Fort Dodge, licensed by the State of Iowa as Residential Care Facilities (RCF). Regulations require dietician-approved menus, physical facility/safety oversight, medication administration/ management, and more. RCF homes provide adults 24 hour support, limited structure, training in independent living skills, and more. Staff employed in the homes work a daily shift.
 

Supported Community Living, Respite

This program provides supports to individuals who live in their own apartments/homes, or in their parental home. People receive training in daily living skills such as housekeeping, cooking, menu planning, budgeting, medication management/administration, managing mental/physical health, and more. 

Respite services are also offered for eligible individuals, with services taking place in the individual’s home.
